Person Basic Information
The Person Basic Information Screen is used to enter general information for a Person such as Social Security Number and Date of Birth.
Screen Items
  • Identification/Social Security Number: use this section to view and/or edit a Person’s Social Security Number (SSN). To edit an SSN, you must first check the Change box.

    The system requires that each Person have a unique number in the database and will validate that there are no other individuals with the same number.
  • Life Status: enter Date of Birth and Date of Death in this section.

    If the date of birth is unknown, check the Unknown box.

    If a Person is still alive, the None box should be checked instead of entering a date of death.
  • Gender: this section is used to define a Person's gender. Select either Male, Female, or Unknown. You can press the blue Lookup button if the gender is unknown to see if the System can determine gender based upon the Person’s first name.

    The numeric equivalent of each choice stored in the system is as follows:

    • Unknown: 0
    • Male: 1
    • Female: 2
  • Comment: add a comment to be viewed when any user opens the Person Basic Information Screen for this Participant.
  • Electronic Consent: this item can only be edited through the Internet Portal by the Participant, and indicates whether they have consented to receive electronic notifications.
  • Alternate Identifier: alphanumeric identifier used to identify Paticipant for clients using Internet Portal Single Sign-On.
  • Sample Participant: check this box to identify this person as a dummied sample.
