The Forms Configuration Screen allows you to set up what online forms are available as well as other options when completing forms for the purposes of completing this Worktrack step.
- Forms Type: choose the type of forms package that users are to complete. While there is currently only one option, Standard Election Forms, this list will uptimately be expanded to allow for different sets of forms.
- Optional Forms: this area allows you to indicate which optional forms should or should not be displayed to portal users.

Allowable Actions:
This section allows you to specify what actions should be displayed and allowed when portal users are completing their forms:
- Payment deferral until MRD: this option is not yet available.
- Online Signature Witnessing: this option is currently not applicable as online signature witnessing is currently not permitted by the IRS.
- Proof of identification upload: check this box if portal users are allowed to electronically upload proof of identification and marriage.
- Notarized forms upload: check this box if users are allowed to upload their notarized forms prior to mailing notarized copies.
- Marriage Certificate issued by Church: this box should be checked if the plan allows for marriage certificates that are issued by a church (in addition to certificates issued by city/town).
- Partial rollover of lump sum: checking this box will allow portal users to rollover a portion of their lump sum.
- Multiple payees/banks: allows for portal users to select a second payee for receiving payment.
- Local withholding: check this box if users can specify local withholding.
- W-4P / W-4R SSN update: enable this option if users can specify Social Security Number (appears on the W-4P / W-4R). By default, the SSN stored in DB Precision will be used.
- W-4P / W-4R name update: enable this option if users can specify their name (appears on the W-4P / W-4R). By default, the name stored in DB Precision will be used.
- W-4P / W-4R mailing address update: enable this option if users can specify their mailing address (appears on the W-4P / W-4R). By default, the mailing address stored in DB Precision will be used.

Other Options:
This section provides additional options for portal users when completing their forms:
- Show group in header: to display group name in addition to plan name, choose the Reported Amount for determining group name.
- Include 'Contingent Annuitant' in J&S descriptions: if the plan includes a "Contingent Annuitant" option as well as "Joint and Survivor" option, check this box.
- Suppress forms not applicable when printing: check this box to only print forms that are applicable based upon each users choices/elections (hides printed forms not applicable).
- Do not show online signature if notary required: check this box if you don not want users to need to digitally sign forms that already require signature in front of a notary.
- Forms Return Date: use this area to specify when forms should be returned in oder to ensure timely payment.
- Mailing Address for Notarized Forms: when notarized documents are part of the forms package, you must specify a mailing address where they can mail original notarized documents.
- Death Benefit Description Override: space to indicate custom death benefit wording (appears on the benefit description pages).
