If PensionSoft is converting a Plan for you, please provide the following:
- Latest Pension Plan Document: This should include all plan amendments (if any).
- Latest Valuation Report: The date of this valuation should be equal to the valuation date that the data provided corresponds with.
- Sample Benefit Statements: Provide in Microsoft Word format if possible.
- Sample Benefit Estimates: If possible, please include as much derivation and/or calculation detail, as this ensures that certain precedents that would not necessarily be found in a Plan document are being followed in DB Precision.
- Sample Election Forms: Provide in Microsoft Word format if possible.
- Participant Valuation Data: This will likely come from your actuary's liability software application, and should be provided as an Excel file. The following data fields should be included in this file:
- For All Participants: Name, gender, date of birth, and valuation status.
- For Current Employees: Employment dates, and any information such as location code, hours, or salary, that may be pertinent in calculating accrued benefits.
- Participants in Receipt of or Due Payment: Benefit amount, payment start date (actual or expected), and form of annuity (including joint and survivor percentages and certain periods, where applicable).
- Participant Benefit Statement Data: Any information such as accrued benefits, projected benefits, and components that are used in calculations (such as service amounts, and average earnings if the plan is earnings-based). Please provide in an Excel format for all participants if possible.
