The Call Center Search tool allows a DBP user to search for participants across multiple plans by entering known information from a caller into the caller information fields. When any information is entered into the caller information fields the find button can be used to search your DBP database for all participants with information matching what was entered. You then can click on a participant in the matches found section to open them up in DBP.
- Last 4 digits of SSN: enter the participant's last four of their social security number. If more than 4 numbers are entered only the last four numbers in the string will be searched for and if less than 4 numbers are entered the search will assume leading zeros.
- First Name: enter the first name of the participant. If a name in the system contains part of the name searched it will appear as well. Example: When searching "Jo" first names like Joshua, Johnathan, and Marjorie will show as valid results.
- Last Name: enter the last name of the participant. If a name in the system contains part of the name searched it will appear as well. Example: When searching "Co" last names like Cobb, Coffman, and Acosta will show as valid results .
- Date of Birth: enter the known date of birth of a participant. An exact date of birth is required if using this search term.
- Company EIN: enter the EIN of the company the participant works under, the lookup button can be used to quickly fill out a company's EIN.
- State/Province: enter the two letter abbreviation of a state or province. The lookup button can be used to find the two-letter state/province code.
- Zip/Postal Code: enter the five digit portion of the participant's zip code.
- Phone: enter the phone number of the participant. The exact number with area code must be entered in the same way it is stored under the participant's address information for this search to work.
